New patients or 3 years since your last visit, please
arrive 30 minutes early to complete your file. Follow-up
visits, please arrive 20 minutes early to update your
files. Be sure to bring a current insurance card with
you to each visit. Also, please bring recent x-rays,
medical reports, and a current list of medications.
We normally call to remind you of your appointment
1-3 days prior to the appointment. Our office uses an
automated system to remind you of the time and date.
This system will normally leave a message on your answering
machine if you are unavailable. Please notify the receptionist
at your appointment time if you do not want to receive
this call.
We ask that when rescheduling, you give at least 24
hrs. notice.

All of our physicians participate with Medicare and
accept assignment. For all insurance plans that require
a co-pay, you will be asked to take care of this upon
check-in. In the event you are unable to pay your portion
at your appointment, please contact our office and speak
to an accounts supervisor for payment arrangements prior
to your appointment date. Please make sure that you
contact your primary care physician to obtain a referral
as soon as your appointment is scheduled, (if your insurance
requires a referral). This will allow your physician’s
office to prepare and provide our office a referral
prior to your appointment time. Please feel free to
contact our office if you have any questions.
We must emphasize that as medical care providers, our
relationship is with you, not your insurance company.
All charges are your responsibility from the date the
services are rendered. Our fees are generally considered
to fall within the acceptable range by most companies,
and therefore are covered up to the maximum allowance
determined by each carrier. Not all services are a covered
benefit in all contracts. Some insurance companies arbitrarily
select certain services they will not cover.
We realize that temporary financial problems may affect
timely payment of your account. If such problems do
arise, we encourage you to contact one of our account
supervisors regarding the management of your account.
